Any data transmitted as part of the appointment booking form is used exclusively for the purposes of managing and making appointments. By completing the form, you agree to be contacted by SVR.

SVR collects your personal data necessary to make your appointment.

The categories of data that SVR may collect are as follows:

• General identification data (title, surname, first name, specialty and professional email);
• Contact details: office address or city and department of practice, telephone number.

We keep your data for a period of 36 months.

In accordance with the provisions in force, you have the right to access, modify, rectify and delete information concerning you.
